Returns (Form A)

• You need only complete this form if you are reporting fundraising for more than one church and/or have undertaken a house-to-house collection.
• Do not include in this form any income raised via digital platforms (like JustGiving or the e-envelope) as we receive those details automatically.
• Return all paperwork (like charity vouchers and Gift Aid declarations) in the pre-paid envelope. You should have received one with your Christian Aid Week materials order, but you can order additional treasury materials if you need them.
